Have you, or someone you care for, been discharged from hospital recently? We want to know about your experiences.


Leaving hospital should be a safe and well-supported step towards recovery. But for some people, the process can be confusing, delayed or leave them without the support they need.

We are working with Healthwatch England to understand people’s experiences of leaving hospital, also known as hospital discharge.


You or someone you care for may have experienced delays while waiting for care, equipment or support. Or perhaps you were discharged sooner than you felt ready for, without the information or help you needed.


Whether you had a positive or negative experience, we want to hear from you. Your feedback can help us understand what is working well and where improvements are needed, both locally and across England.
