Early findings from the national maternity and neonatal investigation

On 26 February 2026, the Independent National Maternity and Neonatal Investigation published an interim report, sharing early findings and recommendations while the full investigation continues.
Background
The investigation was launched in June 2025 by Health Secretary Wes Streeting to review NHS maternity services in England. Led by Baroness Valerie Amos, its goal is to identify the “systemic causes of unacceptable care affecting women, babies, and families.”
The investigation includes local reviews of maternity services at 12 NHS trusts. So far, over 8,000 people and more than 400 families across England have shared their experiences. There is still time to share your experience; see details further down this page.
Why it matters
The investigation was set up after some high-profile cases of maternity failings, highlighting the urgent need to improve care and safety for mothers and babies.
The interim report reveals serious and long-standing issues, including:
Insensitive/uncaring staff
Racism and discrimination
Chronic staff shortages
Attempts to cover up mistakes
Lack of transparency when families seek answers about trauma or loss

Next steps
The investigation concludes in Spring with two final reports expected. These will include detailed recommendations aimed at improving maternity and neonatal care across England.
